Immunizations Certificate of Immunization

  • Diphtheria/Pertussis/Tetanus (DPT/TD) - 5 Doses

  • A booster of Tdap is required for 7th grade

  • Polio Vaccine - 4 Doses before entrance to Kindergarten and Grade 7

  • Measles/Mumps/Rubella (MMR*)

    • 1 Dose before entrance to Pre-K

    • 2 Doses before entrance to Kindergarten and Grade 7

      * or official documentation of disease/immunity             

  • Hepatitis B

    • 3 Doses before entrance to Kindergarten and Grade 7

  • Varicella*

    • 1 Dose before entrance into Pre-K

    • 2 Doses before entrance into Kindergarten and Grade 7
      * or official documentation of disease/immunity

  • Meningococcal - MenACWY (formerly MCV4)

    • Grades 7-9 require 1 Dose

    • Grades 11-12 require 2 Doses 

  • Tuberculin Test as required by Board of Health Criteria

  • Lead Screening is required for entrance into Kindergarten

     

A private physician's examination, which includes immunization information, is required for all students upon entrance into K, 4, 7, 10 and all transfer students and any student who is participating in a school sponsored sport at the middle or high school level.

Mandatory Health Screening Programs

  1. Physical Examination: Due at the beginning of the school year for grades K, 4, 7 and 10. All students must have a current Physical Examination on file (within the last 365 days) before participating in any sport.

  2. Lead screening: The Department of Public Health regulations require that all children entering kindergarten present documented evidence of lead screening.

  3. Vision: Done annually in the fall in grades K-5, 7 and 9.

  4. Hearing: Done annually in the fall in grades K-3, 7 and 9.

  5. Postural Screening: Done annually in the spring in grades 5-9.

  6. Height, Weight and BMI Calculations: Done annually in grades 1 ,4, 7 and 10.

  7. SBIRT Screening: Done annually in the fall in grades 7 and 9.
